Services Care Management The LifeSpan Care Management staff consists of 28 dedicated professionals. Each is responsible for monitoring approximately 50-60 consumers.  Eligibility requirements:  60+, living in Allegheny County, and in need of any of the following services: Personal Care Home Delivered Meals Telephone Reassurance Senior Companion Program Caregiver Relief Program Adult Day Care Volunteer Opportunities Frozen Meal Program Community Services The LifeSpan Community Services Program of LifeSpan offers many an opportunity for both social and recreational activities at our thirteen senior community centers and one community extension program (Century III). These services include, but are not limited to: PrimeTime Health Program Transportation Registration Congregate Meals Entitlement Programs Neighborhood Legal Services Life Enrichment Classes Information & Referral
