Getting Started:

Q:  What services can I expect to be provided for my elderly parent through LifeSpan's Care Management Department?
A:  A Home visit with a Care Manager, including discussions on personal care (bathing, dressing, grooming), light housekeeping, nutrition and incontinence supplies, senior companion services, MOW/HDM, small emergency home repairs, referrals to other resources including home safety devices/medication management and emergency response units.

Q:  Do you have someone available to escort my parent to the doctor or shopping?
A:  No, but we can get your parent registered for Access/OPT or make referrals to programs that can potentially assist.

Q.  What is the eligibility criteria for an individual to receive services through LifeSpan, Inc.?
A:  Individual must be a resident of Allegheny County and reside within LSI's service area.  They must be 60+ years of age.  Husband and wife residing in the same household can both receive services.  

Q:  How does an individual get the process started?  What information do I need to have in hand?
A:  A call to the Allegheny County Area Agency on Aging Senior Line (412-350-5460) or (800-344-4319). This is the point of entry for services. Have basic identifying information available, i.e., proof of age, birthday.You will also be asked for medical conditions and needs - why you feel you need the services. LifeSpan is one of several providers of care management services in Allegheny County and the AAA assigns cases according to provider service areas. Anyone can make a referral for services on another person's behalf, but the individual must first consent over the phone and then in writing.

Q:  Some individuals may not want to share their financial information during this process - can they still apply for services?
A:  Most services require proof of income, and some require income and asset information, but there are services that are not income based, i.e., Care Management, Home Delivered Meals and Senior Companion program.

Q:  Is a personal visit necessary for the first interview? Should a family member be present? Who will conduct the first interview? How do we know this person can be trusted?
A:  A face-to-face visit is mandatory.  The individual can have anyone present during the interview that they chose - but it is not required. A Care Manager will conduct the first interview,  All LifeSpan care managers have criminal clearances.  Please do not hesitate to ask for ID.

Q:  I overheard my Dad on the phone with LifeSpan and he is telling a very different story about his situation than is true.  Can I speak to his care manager about the situation?
A:  Yes, you can speak on your father's behalf, but the care manager must have his permission since LSI follows HIPAA Guidelines.

Q:  Who actually decides which services an individual should receive?  Will a menu of services be given to the individual and/or family for review? 
A:  The care manager will recommend services for which an individual is eligible depending on the circumstances.  The care manager works in collaboration with the individual and the family and orders services based on need.  For example, if the individual wants services for housekeeping, but they have an able-bodied person in the home to do this - they may not receive housekeeping services.

Q:  If an individual does not qualify financially for coordination of services is it possible to private pay for this service coordination?  For example, the entire family lives out of state and it would really help if one agency could coordinate all services.
A:  Yes.

Q:  If an individual is eligible for Veteran's Benefits can they receive services from both the VA and AAA?
A:  Yes.  The care managers can help individuals apply for Veteran's benefits. LifeSpan and the AAA cannot duplicate services, but we can work in conjunction with the VA and other insurance to ensure needs are met.

Q:  Mom is afraid that if someone comes into her home they will say that she needs to go into a nursing facility and she will not be allowed to remain in her home.  How do we handle this?
A:  It is our goal to keep individuals in their homes and communities as long as they can safely live there.
