Caregiver Support Program

The Caregiver Support Program (CSP) assists families caring for an older relative or friend.  The goal being to support the care provided to frail family members in their home.  The focus is to respond to the needs of caregivers, so they can provide the best possible care for an older relative or friend in the home.

Assistance completing benefits and insurance forms
Reimbursement of qualifying caregiving expenses

OPTIONS
Care Management Program

The OPTIONS Care Management Program is an assessment service provided by the Area Agency on Aging, through LifeSpan, Inc. to help older adults, their family and other members of their support network, to determine what long-term living services best fit their needs.

Participants who receive these services are normally 60 years of age or older and experience some degree of frality in their physical and/or emotional health.  They range in need from being eligible for a nursing facility to just needing some basic support services such as help around the home or personal care.

There is no financial eligibility requirement for OPTIONS based services.  However, consumers may be required to make a co-payment for services based on income.
 

Protective Services

Are you concerned that you or a loved one may be a victim of abuse?
Investigate elder abuse and neglect in the community, facilities and  hospitals.
Provide assistance to alleviate imminent risk. 
Provide a full evaluation to determine the needs of older adults.
Retain an older adult's autonomy throughout the process of investigatgion, if they are determined to be "of sound mind".
Coordinate services with other entities to assist in the long term care of seniors.
