The OPTIONS Care Management Program is an assessment service provided by the Area Agency on Aging, through LifeSpan, Inc. for older adults to help them, their family and other members of their support network, to determine what long-term living services best fit their needs.
Participants who receive these services are normally 60 years of age or older and experience some degree of frailty in their physical and/or emotional health. They range in need from being eligible for a nursing facility to just needing some basic support services such as help around the home or personal care.
There is no financial eligibility requirement for OPTIONS based services. However, consumers may be required to make a co-payment for services based on income.
